One of the best ways we can serve our customers is by ensuring access to a high quality provider network. However, please note that many Washington counties are closed to accepting new providers. Before applying, be sure to check the
Washington Network Closure Matrix.
Update: Effective June 1, 2018, Washington House Bill 2335 requires all insurance carriers to use ProviderSource, which LifeWise
currently uses. The bill also requires providers to submit credentialing applications using OneHealthPort's secure login.
Also effective June 1, LifeWise must approve or deny the complete application no later than 90 days after receipt. A complete application is one that is not missing any information. This requirement changes to 60 days on June 1, 2020.
You can request an individual contract or join one of our contracted independent practice associations or medical groups. In either case, you must also meet our credentialing criteria:

The top reasons for incomplete applications include:
PO Box 327 - MS 263
Seattle, WA 98111-0327
The credentialing process typically takes 60 to 90 days. Your acceptance is based on application information and network requirements. If your submitted application is complete and you're accepted, you'll receive a counter-signed contract and start date. For all LifeWise plans, you can't see members
under the contract until you're officially credentialed. Also, your malpractice insurance must start before credentialing is complete.
